SCSHP Awards Criteria and Nomination

AWARDS

The South Carolina Society of Health-System Pharmacists (SCSHP) annually conducts an awards program recognizing outstanding current SCSHP members who have made significant contributions to the practice of pharmacy in the state of South Carolina. Active SCSHP members may be nominated for the awards (listed below) by completing the online Awards Nomination Form.  All nomination letters for all awards should be submitted by an SCSHP member. Unless otherwise indicated, all award presentations will be made at the SCSHP Annual Meeting. Deadline: January 31st.

Current executive board members are not eligible to receive SCSHP awards.

PAI BEST PRACTICE AWARD

SCSHP is looking for pharmacists across the state to submit projects or initiatives implemented that relate to one or more of the 5 focused areas of the Practice Advancement Initiative 2030. 

Submissions will be considered for a panel presentation on PAI at the Annual Meeting where the award will be presented. 

PAI 2030 focused initiatives:


  1. Optimize medication use and access through pharmacist prescribing.
    Objective: By 2024, increase authority for pharmacists to independently prescribe medications, pursuant to a diagnosis, by 20%.
  2. Leverage and utilize technology to optimize pharmacist provision of care to patients.
    Objective: By 2024, increase by 20% the number of health systems using analytics and technology to reduce the risk of adverse drug events or suboptimal outcomes.
  3. Ensure all patients receive seamless and coordinated pharmacy services at all transitions of care.
    Objective: By 2024, increase the number of health systems with integrated pharmacy services to coordinate effective patient care transitions by 20%.
  4. Improve patient access to pharmacist services in ambulatory care clinics.
    Objective: By the end of 2024, increase the number of pharmacists in ambulatory care clinics by 20%.
  5. Expand the role of pharmacy technicians.
    Objective: By 2024, increase the number of health systems utilizing pharmacy technicians in advanced roles by 20%.

PAI Best Practice Award Nomination Form

SCSHP PHARMACIST OF THE YEAR AWARD

The award is given annually to an active health systems pharmacist with sustained achievements and contributions to health-systems pharmacy and/or involvement with the Society. The award is limited to one recipient per year.

Criteria:

The nominee must be a practicing pharmacists in the state of SC for greater than or equal to 5 years as well as an active member of SCSHP for greater than or equal to 5 years and actively involved with the Society. Examples of involvement may include: planning and/or moderating CE programs, speaking engagements, serving as an ASHP delegate, recruiting new members, serving as a faculty advisor, promoting and encouraging membership to the Society, or chairing a committee. Nominees should have sustained achievements and/or contributions to health-systems pharmacy. Nominees must have improved health-systems pharmacy practice and/or the Society through their contributions to the profession, such as participating in Society-sponsored events (local, state, or national), public involvement and awareness; stimulating interest of the Society to non-members; promoting legislative involvement through local, state, or national representation for the pharmacy profession. Nominees shall have performed pharmacist tasks at a health-system site in an extraordinary fashion as detailed by the member making the nomination. Nominees must be licensed in the state of South Carolina. Nomination packets must include a letter from the nominator to the Nominations and Awards Committee detailing the nominee's qualifications for this award, the nominee's curriculum vitae, and up to two letters of support from a SCSHP member or non-member of the Society.

Pharmacist Of The Year Nomination Form

SCSHP YOUNG PHARMACIST OF THE YEAR AWARD

The award is given annually to an active health systems pharmacist with extraordinary achievements and contributions to health-systems pharmacy and/or involvement with the Society. The award is limited to one recipient per year.

Criteria:

The nominee must be licensed as a pharmacist for less than 5 years, practice in the state of SC, and be an active pharmacist member of SCSHP for less than 5 years. Examples of involvement may include: planning and/or moderating CE programs, speaking engagements, serving as an ASHP delegate, recruiting new members, serving as a faculty advisor, promoting and encouraging membership to the Society, or chairing a committee. Nominees should have extraordinary achievements and/or contributions to health-systems pharmacy. Nominees must have improved health-systems pharmacy practice and/or the Society through their contributions to the profession, such as participating in Society-sponsored events (local, state, or national), public involvement and awareness; stimulating interest of the Society to non-members, promoting legislative involvement through local, state, or national representation for the pharmacy profession. Nominees shall have performed pharmacist tasks at a health-system site in an extraordinary fashion as detailed by the member making the nomination. Nominees must be licensed in the state of South Carolina. Young Pharmacist Of The Year Nomination Form

SCSHP TECHNICIAN OF THE YEAR AWARD

The award is given annually to recognize an active technician member for his or her contributions to health-systems pharmacy practice and/or the Society. The award is limited to one recipient per year.

Criteria:

The nominee must be a state Certified Pharmacy Technician for a minimum of 2 years and must be a member of SCSHP for a minimum of 2 years. Nominees must have performed technician tasks at a health-systems site in extraordinary fashion as detailed by member making nomination. Nominees must have made a significant contribution to health-systems pharmacy practice and/or SCSHP, such as participating in Society-sponsored events (local, state, or national), public involvement and awareness; stimulating interest of the Society to non-members; and promoting legislative involvement through local, state, or national representation for the pharmacy profession. Nominees must be licensed in the state of South Carolina. Nomination packets must include a letter from the nominator to the Nominations and Awards Committee detailing the nominee's qualifications for this award, the nominee's curriculum vitae, and up to two letters of support from a SCSHP member or non-member of the Society.

Technician Of The Year Nomination Form

SCSHP KENNETH FLINCHUM DISTINGUISHED SERVICE AWARD

The award is presented when appropriate, but not necessarily annually to an individual who has made exceptional and sustained contributions to health-systems pharmacy practice and SCSHP.

Criteria:

The nominee must be a practicing pharmacist in the state of SC for more than 10 years as well as an active member of SCSHP for more than 10 years. Nominees must have provided at least 10 years of "extraordinary” service to health-systems pharmacy and/or SCSHP. Years of membership and/or extraordinary service do not need to be consecutive years. Extraordinary service is defined as activities that have been critical in advancing health-systems pharmacy practice and/or SCSHP’s mission. Nominees must have improved health-systems pharmacy practice and/or the Society through their contributions to the profession. Nomination packets must include a letter from the nominator to the Nominations and Awards Committee detailing the nominee's qualifications for this award, the nominee's curriculum vitae, and up to three letters of support from a SCSHP member or non-member of the Society.

Kenneth Flinchum Distinguished Service Nomination Form

SCSHP GEORGE D. SCHWERIN MENTOR AWARD

The award is presented when appropriate, but not necessarily annually to an individual in recognition of their extraordinary mentorship of student pharmacists, pharmacy technicians, pharmacy residents, pharmacists and/or other members of the healthcare team. The award is limited to one recipient per year.

Criteria:

The nominee must be a practicing pharmacist in the state of SC for more than 5 years as well as an active member of SCSHP for more than 5 years. Nominees must have mentored student pharmacists, pharmacy technicians, pharmacy residents, pharmacists and/or other members of the healthcare team in professional, supervisory, academic, or peer relationships; contributed to the profession by inspiring these individuals to reach their potential and beyond; and/or educated and trained these individuals for health-systems practice. Nominations must include a letter from the nominator to the Nominations and Awards Committee detailing the nominee's qualifications for this award, the nominee's curriculum vitae, and up to two letters of support from a SCSHP member or non-member of the Society.

George D. Schwerin Mentor Nomination Form

SCSHP OUTSTANDING STUDENT AWARD

The award is presented annually to a student pharmacist within the state of South Carolina for his or her achievements and/or contributions to the local student chapter and the Society. One nomination will be accepted from each pharmacy school campus in South Carolina. The award is limited to one recipient per year.

Criteria:

Nominees must be a member of the local chapter and SCSHP for at least 2 years. Nominees must have actively participated on a student chapter level but also state level to promote health-systems pharmacy and the profession. Leadership and service in other student organizations and/or community service will be highly considered towards this award. Nominees must be in good academic and professional standing at the school of pharmacy, holding a GPA of 3.5 or higher. Nominees should be in good standing with local chapter and the Society and should not have received the award in previous years. Holding a Junior Board position or other leadership position is not a requirement for this award. Nominations must include a letter from the Dean and/or faculty advisor. The nomination letter for this award must state that the student is in good professional and academic standing and must confirm the student's cumulative GPA following the most recent grading period.The nomination must also include the nominee's CV and an optional letter of support.

Outstanding Student Nomination Form

SCSHP OUTSTANDING STUDENT CHAPTER AWARD

This award is given annually to recognize a South Carolina SSHP chapter for their contributions to health-systems pharmacy. This award is limited to one chapter per year.

Criteria:

• The nominated chapter must be listed on ASHP’s list of officially recognized student chapters
• Chapter activities should:
- Promote membership in local, state, and national health-system organizations 
- Stimulate interest in health-system pharmacy careers
- Encourage career development and professionalism among students aspiring to careers in health-system pharmacy
• The nominated chapter must participate in a project that supports the mission of SCSHP and/or ASHP.
• The nominated chapter must also demonstrate a commitment to community service. 

The nomination packet should include activities from the spring and fall semesters preceding the award nomination deadline. The nomination packet must include 1) a listing and description of the chapter’s career development activities, 2) a listing and description of the chapter’s community service activities, 3) the professional development project submitted for ASHP recognition the previous year, including a copy of the poster presented at the ASHP Student Chapter Showcase, and 4) any additional information regarding the chapter’s activities that support the nomination (e.g., member involvement in state and national meetings – SCSHP, ASHP, Pharmacy Day at the Statehouse, etc.; member involvement in state and national leadership positions and committees; etc.).

Outstanding Chapter Nomination Form

SCSHP STUDENT SCHOLARSHIPS

Scholarships will be presented annually to student pharmacists within the state of South Carolina. Nominations will be accepted from each pharmacy school campus in South Carolina. Four scholarships will be awarded per year.

Criteria:

Applicants must be a member of the local chapter and SCSHP for at least 1 year. Nominees must have actively participated on a student chapter level but also state level through events and meetings. Nominees must be in good academic and professional standing at the school of pharmacy, holding a GPA of 3.0 or higher. Holding a Junior Board position or other leadership position is not a requirement for a SCSHP scholarship. Nominations must include a letter from the Dean and/or faculty advisor. The nomination letter for this award must state that the student is in good professional and academic standing and must confirm the student's cumulative GPA following the most recent grading period. The nomination must also include the nominee's CV and an optional letter of support. Applications must include an essay up to 500-words detailing contributions to the local student chapter and state organization.
